OBTAINING DOCUMENTS FROM SERBIA

PROOF OF LIFE CERTIFICATE 

 

 

 

TRANSPORT OF HUMAN REMAINS TO SERBIA
 
Transport of human remains, either decedent’s body or cremated ashes requires a permit called SPROVODNICA; the following documents and procedures are required:

  1. Completed request of the family (MS Word Form or PDF Form) + coppy of the ID and relation of the family member requesting the document  
  2. A valid passport of the Republic of Serbia belonging to the deceased,
  3. Original death certificate, certified by Apostille seal (document is certified by the Secretary of State - Office for the Authentication of Documents),
  4. *Statement that the deceased did not die of any contagious disease, issued by the local health authority,
  5. Burial-transit permit issued by American authorities for transport of the human remains to the Republic of Serbia.
  6. *Certificate issued by a Funeral Home stating that the human remains were not previously buried, that they were prepared for transport according to the international regulations and placed in a hermetically sealed metal/zinc box,
  7. *A statement from the Funeral Home as proof that the deceased person was identified and that casket contains nothing but the remains of the named deceased,
  8. A letter from the Funeral Home stating the flight itinerary, place (town) to which the remains are being shipped, point of entry into Republic of Serbia and full name of the escort, if any,
  9. Consular fee of $9.00 payable in cash, by card, or by Money Order issued to the Consulate General of the Republic of Serbia,
  10. Stamped, self-addressed return envelope with pre-paid return postage (envelope is required if applying by mail).

In addition:

  • If the deceased is not a citizen of Republic of Serbia, it will be necessary to present a foreign identification document and a certificate of the burial place provided by the cemetery in Republic of Serbia.
  • If the cause of death was a plague or smallpox Sprovodnica may be granted only with the consent of the Ministry of Health of the Republic of Serbia.
  • If the remains are to be only transported through the territory of Republic of Serbia, beside above listed documents a permit (Sprovodnica) from the country of burial must also be obtained before the transportation is permitted.
  • Numbers 4,6 and 7 with an asterisk do not refer to transfer of the urn.

 

OBTAINING DOCUMENTS FROM SERBIA

If you require documents from the Republic of Serbia, the Consulate General can obtain the following documents:

  • Birth Certificate (excerpt from the Register of Vital Statistics – Births) (MS Word or PDF  format)
  • Marriage Certificate (excerpt from the Register of Vital Statistics – Marriages) (MS Word or PDF  format)
  • Death Certificate (excerpt from the Register of Vital Statistics – Deaths) (MS Word or PDF  format)
  • Proof of Citizenship ( MS Word  or  PDF  format)
  • Police Clearance (MS Word or PDF format)

Annotations:

  • Along with the request one must submit photocopies of all available documents from the Republic of Serbia,
  • If the request is submitted by mail one must enclose a pre-stamped, pre-addressed return envelope with adequate postage,
  • Consular fee for obtaining a Birth Certificate (domestic form) is $64.00 and is paid in cash or by Money-Order issued to Consulate General of the Republic of Serbia. The fee is non-refundable.
  • Consular fee for obtaining a Certificate of Proof of Citizenship of the Republic of Serbia is $68.00 and is paid in cash or by Money-Order issued to Consulate General of the Republic of Serbia. The fee is non-refundable.
  • Consular fee for obtaining a Police Clearance is $ 60 and is payable in cash, by card, or by Money Order issued to Consulate General of the Republic of Serbia in Chicago. The fee is non-refundable.

PROOF OF LIFE CERTIFICATE 

 

In order to exercise the right to social security Consulate General may issue a Proof of Life Certificate to the citizens of the Republic of Serbia who reside in the consular district of the Consulate General in Chicago.

The following is required for the issuance of the certificate;

  • Personal presence. If a request is submitted by not coming to the Consulate
  • General, it is necessary that the request be signed before the Notary Public;
  • A completed application form for the issuance of the Proof of Life Certificate;
  • Valid passport of the Republic of Serbia or other valid identification document of the Republic of Serbia (identity card, driving license);
  • Consular fees in the amount of $4.00 payable in cash, by card, or Money Order issued to the Consulate General of the Republic of Serbia in Chicago
  • Pre-addressed return envelope with prepaid postage if the request is sent by mail.
