Transport of human remains

BURIAL TRANSPORT PERMIT

Burial Transport Permit is a permit necessary for transport of human remains (casket or urn) for burial in the Republic of Serbia, or for transit of human remains trough the Republic of Serbia.

Required documents:

1.       completed Burial Transport Permit request form.

2.       a valid passport of the Republic of Serbia of the deceased (passport is cancelled and returned to the family).

3.       original death certificate, certified by Apostille.

4.       *statement that the deceased did not die of any contagious disease, issued by the local health authority.

5.       burial-transit permit issued by U.S. authorities for transport of the human remains to the Republic of Serbia.

6.       *certificate issued by a Funeral Home stating that the human remains were not previously buried, that they were prepared for transport according to the international regulations and placed in a hermetically sealed metal/zinc box.

7.       *a statement from the Funeral Home certifying that the deceased person was identified and that casket contains nothing but the remains of the named deceased.

8.       a letter from the Funeral Home stating the flight itinerary, point of entry into the Republic of Serbia, place (town) to which the remains are being shipped, and full name of the escort, if any.

9.       Numbers 4, 6 and 7 with an asterisk are not applicable to transfer of the urn. Please provide Cremation Certificate instead.

 

Consular fee of $4.00 USD, payable by card, cash and MoneyOrder made out to the Consulate General of Serbia in Chicago (personal checks are not accepted).

If applying by mail, please include a self-addressed return envelope with pre-paid return postage.

·         If the deceased was not a citizen of Republic of Serbia, a foreign passport of the deceased and a certificate of the secured burial plot in Republic of Serbia are required.

·         If cause of death is contagious  disease, prior approval by the Ministry of Health of Serbia is required.

·         If Serbia is a transit country for the transport of the remains, make sure to obtain Burial Transport Permit from the country of burial first.
